How To use the pharmacy cards

Retail pharmacy

  1. Print this card.
  2. Show this card to your pharmacist with your prescription(s).
  3. Save an average of 15-55% on your prescriptions.

Tip : The pharmacy should keep your information on file in their computer system. Keep this card in a safe place in case you change pharmacies or if you need to provide this information again.

Mail-order pharmacy

  1. Price your brand-name medication.
  2. Call 855-633-7977 to place your order or refill.
  3. Your medications ship directly to you.

This plan is a discount plan NOT a health insurance policy. This plan provides discounts at certain health care providers for medical services.
